Our clinical, COACH and group programs are FREE for people with, or at risk of diabetes. Individual consultations are offered via telehealth, video as well as face to face. One of our health professionals will contact the patient and talk to them about how we can help.
With the patient’s consent, please complete the referral form (links to Primary Health Tasmania website) and attach the relevant medical history.
Instructions for downloading our referral template onto your Best Practice or Medical Director practice software are available on the PHT website (a pdf version of the referral form is also available).
